"Lie still, darling. The fever passes quicker when the blanket is tucked in." โ Sister Halvek's final note
Lore & Description
Appearance
This enormous quilt is assembled from mismatched hospital blankets: yellowed wool, faded blue cotton, rough gray burlap, and squares stained brown with age. Its seams writhe like veins, and its corners fold into grasping hands without fingers. Names are embroidered across its surface in red, black, and sickly green thread, some crossed out and others still wet with blood. Heat radiates from its layers, carrying the scents of fever, camphor, and damp linen.
Behavior & Origins
The quilt was sewn by exhausted attendants in Saint Orra's plague ward, each square cut from the bedclothes of a patient who died there. When the ward was abandoned, the quilt remained warm and hungry, animated by the final breath of everyone who had been wrapped in it. It keeps a list of the living who entered the ward, stole from its sealed reliquary, or failed to mourn the dead. The party's names have recently appeared in fresh crimson thread because Sister Halvek, the last surviving nurse, believes they possess the reliquary's missing silver bell. The quilt stalks the ruined ward at night, seeking to wrap every name on its list.
Running Plague-Ward Quilt
Tactics
The quilt opens by naming the most dangerous-looking character and immediately trying to Smother them, using its reach to keep other characters at a distance. It uses Shake Out when surrounded or when several targets are clustered, then focuses its listed target while feeding on any creature it has grappled. If reduced below 15 hit points, it retreats through narrow passages toward the plague ward's abandoned nursery, but it fights to the death if someone carries the silver reliquary bell.
DM Tips
- Telegraph its presence with warm footprints, beds tucked neatly despite being abandoned, and fresh names appearing in red thread on old blankets.
- Have the quilt whisper patient names in a soft bedside voice; when it chooses a character, it should pronounce that character's full name before attacking.
- Let clever characters distract it by adding or erasing a name from its embroidered list, potentially causing it to change targets for a round.
- For a weaker party, reduce its hit points to 36 and lower Smother's escape DC to 12; for a stronger party, give it two additional Quilt Lash attacks during its first turn.
Loot
- The Ward List — A strip of embroidered blanket bearing the names of the dead, the missing, and several living people connected to the plague ward. One name has been sewn in recently and may reveal the party's next enemy.
- Saint Orra's Brass Bed-Tag — A tarnished brass tag stamped with a ward number. It opens a locked cabinet in the ruined hospital and bears a faint warmth when held near the quilt.
- Fever-Warm Batting — A fist-sized bundle of clean inner stuffing that remains pleasantly warm for a year. A healer can use it as a component for a poultice that grants advantage on one saving throw against disease.
