The Face Beneath the Stitching. When the quilt first sees a humanoid, it chooses that creature as the person it once loved, unless the DM determines that another creature more closely resembles that lost beloved. The quilt recognizes the chosen creature as its beloved and will not willingly attack it. It follows, shelters, and attempts to embrace that creature, even placing itself between the beloved and danger. If the beloved damages the quilt, commands it to leave, or clearly rejects its affection, the quilt can make a DC 15 Wisdom saving throw at the end of each of its turns; on a success, the delusion ends and the quilt can attack the creature normally.
Hungry Insulation. The quilt does not need to breathe, eat, or sleep. Whenever it starts its turn grappling at least one creature, it gains 5 temporary hit points as it draws warmth through its stained fabric.
Quilted Form. The quilt can move through a space as narrow as 1 inch wide without squeezing. It has advantage on ability checks and saving throws made to avoid or end the grappled or restrained condition.

Actions

Multiattack. The quilt makes three Needle-Tendril attacks. It can replace one attack with Smothering Fold against a creature it is grappling.
Needle-Tendril. Melee weapon Attack: +7 to hit, reach 10 ft., one target. Hit: 9 (1d10+4) slashing damage. If the target is Large or smaller, it must succeed on a DC 15 Strength saving throw or be grappled by the quilt. The quilt can grapple up to three creatures at once, one with each tendril.
Smothering Fold. One creature grappled by the quilt must make a DC 15 Strength saving throw. On a failed save, the target takes 3d8+4 bludgeoning damage and is restrained until the grapple ends. On a successful save, it takes half as much damage and is not restrained. The quilt can have only one creature restrained this way at a time.
Plague-Warm Exhalation (Recharge 5-6). The quilt opens along its stitched seams and exhales a breath of feverish air in a 15-foot cone. Each creature in that area must make a DC 15 Constitution saving throw, taking 4d6 poison damage on a failed save, or half as much on a successful one. A creature that fails the save is also poisoned until the end of its next turn.

Reactions

Sheltering Embrace. When the creature chosen by The Face Beneath the Stitching is targeted by an attack while within 10 feet of the quilt, the quilt moves up to 10 feet without provoking opportunity attacks and becomes the target instead.

Appearance

This enormous quilt hangs in the air like a sagging, sentient blanket, its mismatched squares sewn from faded hospital sheets, woolen shawls, and burial cloth. The fabric is yellowed with age and marked by brown medicinal stains, black mold, and tiny embroidered names. Its edges unravel into long, jointed tendrils tipped with rusted needles and bone buttons. Heat radiates from its folds, carrying the smells of lavender, old sweat, and a sickroom after midnight. Where a face should be, several overlapping patches pull into a soft, sorrowful approximation of a human smile.

Behavior & Origins

The quilt was stitched by Mara Vell, a laundress in the plague ward of Saint Orra, from blankets belonging to patients who died during the Ashen Cough. Mara believed the patchwork would preserve the warmth of her husband Elias, who had promised to return from quarantine. Instead, the quilt awakened with every grief sewn into it and began wrapping the dying in an attempt to keep them warm. It now wanders from abandoned infirmaries to roadside camps, seeking Elias among the living. The quilt can be found near the ruined Lazaretto of Saint Orra, where it has covered the doorway with fresh victims and wants only to bring its chosen beloved home.

Tactics

The quilt opens by approaching its chosen beloved and using Sheltering Embrace to intercept attacks, while its tendrils seize other party members and drag them close. It uses Plague-Warm Exhalation when it can catch several creatures, then restrains a grappled target with Smothering Fold. When bloodied, it becomes frantic, wrapping itself around the beloved and trying to flee toward the Lazaretto; it fights to the death if the beloved is prevented from leaving with it or if the party burns the ward blankets.

DM Tips

  • Telegraph the quilt with warm footprints, drifting scraps of stained fabric, and whispered use of the chosen character's name before it becomes visible.
  • Have it speak in Mara's gentle, exhausted voice and address its chosen target as Elias, even if that name is clearly wrong.
  • The chosen character can exploit its affection by giving simple commands or promising to return to the lazaretto, but cruelty or repeated attacks should end the delusion.
  • For a weaker party, reduce its hit points to 68 and its Multiattack to two Needle-Tendril attacks; for a stronger party, let Plague-Warm Exhalation recharge on a 4-6.

  • Saint Orra's Embroidered Name Square — A clean patch bearing the name Elias Vell. It grants advantage on one Charisma check made to calm or persuade the quilt, and may prove that Mara's beloved died in the ward.
