The Face Beneath the Stitching. When the quilt first sees a humanoid, it chooses one humanoid it can see as the likeness of the person it once loved. The quilt recognizes that creature as its beloved until the creature dies or leaves its sight for 24 hours. The quilt won't willingly attack its beloved and has advantage on Wisdom (Perception) checks and saving throws made to locate or reach that creature. Its beloved can use an action to make a DC 13 Charisma (Persuasion) check; on a success, the quilt becomes docile for 1 minute, provided it isn't attacked.
Fever-Bed Warmth. A creature that starts its turn grappled by the quilt or within 5 feet of it must succeed on a DC 13 Constitution saving throw or have disadvantage on its next attack roll before the end of its turn as feverish heat and the sour smell of sickness cloud its senses. The quilt sheds dim orange light in a 5-foot radius.
Patchwork Persistence. The quilt has advantage on ability checks and saving throws made to avoid being shoved, knocked prone, or forcibly moved.

Actions

Multiattack. The quilt makes two Quilt Lash attacks. If it has a creature grappled, it can replace one Quilt Lash attack with a Smothering Fold attack.
Quilt Lash. Melee weapon Attack: +5 to hit, reach 10 ft., one target. Hit: 7 (1d8+3) bludgeoning damage. The lash is a flailing strip of blanket, its seams popping like knuckles.
Smothering Fold. Melee weapon Attack: +5 to hit, reach 5 ft., one Medium or smaller creature. Hit: 10 (2d6+3) bludgeoning damage. The target is grappled (escape DC 13). Until this grapple ends, the target is restrained, blinded, and unable to breathe. The quilt can grapple only one creature at a time. If the target is its beloved, the quilt deals no damage with this attack and instead attempts to hold them tenderly.
Ward-Steam (Recharge 5-6). The quilt releases a cough of hot, damp air and old medicinal vapor in a 15-foot cone. Each creature in that area must make a DC 13 Constitution saving throw, taking 10 (3d6) poison damage on a failed save, or half as much damage on a successful one. A creature that fails the save is poisoned until the end of its next turn.
Gather the Blankets. The quilt pulls loose bedding, clothing, or cloth objects within 10 feet toward itself. One unattended cloth object weighing no more than 20 pounds is drawn to the quilt and sewn into its body, and the quilt regains 5 hit points. This action can't restore the quilt above half its hit-point maximum.

Reactions

Protective Cover. When the quilt's beloved is hit by an attack while within 10 feet of the quilt, the quilt interposes part of its body. The beloved gains a +2 bonus to AC against that attack, potentially causing it to miss. The quilt then takes 3 (1d6) slashing damage as the impact tears its stitching.

Appearance

This enormous quilt hangs together like a sagging, many-colored corpse. Its squares are cut from hospital blankets, children's afghans, military bedrolls, and strips of burial cloth, all stitched with black surgical thread. Damp heat rolls from its seams, and amber light glows beneath the fabric as though a feverish body were trapped inside. Patches bear faded initials, blood-brown stains, embroidered flowers, and crude ward numbers. When it moves, its corners drag across the floor like grasping hands, while its folds open and close in a silent imitation of breathing.

Behavior & Origins

The quilt was made during the Blackbell Fever from every blanket left behind in Saint Orra's plague ward. A nurse named Elian stitched the blankets together while caring for his dying partner, Mara, and whispered promises into every seam. When the ward was abandoned, the quilt absorbed the last warmth, breath, and grief within it. Now it wanders through ruined infirmaries and tenements, seeking Maraโ€”or anyone whose face, voice, or kindness reminds it of her. The creature currently haunts the quarantined district of Bellwether Row and wants to bring its chosen 'Mara' home to the sealed ward where it believes the two of them can finally rest.

Tactics

The quilt opens by approaching its chosen beloved and trying to wrap them in a Smothering Fold, while using Ward-Steam against anyone who interferes. It positions itself between the beloved and the party, using Protective Cover whenever possible and dragging grappled victims toward dark corners or abandoned beds. When reduced below half its hit points, it frantically uses Gather the Blankets and lashes at whoever threatens its beloved; it fights to the death if that creature remains nearby, but retreats into narrow bedding-filled spaces if the beloved leaves its sight.

DM Tips

  • Telegraph the creature with warm footprints, damp hospital smells, blankets folded into human shapes, and whispered repetitions of the name 'Mara' before combat.
  • Play its voice as muffled speech from several fabric layers, alternating between tender bedside murmurs and sudden wet coughing.
  • Let the chosen character exploit its delusion: a convincing promise to return to Saint Orra's ward can pause the fight long enough for negotiation, escape, or a more favorable position.
  • For a weaker party, remove Ward-Steam's recharge or reduce its damage to 7 (2d6); for a stronger party, give it 10 temporary hit points whenever it uses Gather the Blankets.
