Soul Ledger. Whenever a creature touches the ledger or willingly takes it from another creature, that creature must succeed on a DC 13 Wisdom saving throw or become audited for 24 hours. An audited creature takes 3 (1d6) psychic damage, and The Unclosed Account knows the creature's direction and distance from it while they are within 1 mile of each other. The creature can repeat the saving throw whenever it finishes a long rest, ending the effect on a success. A creature that succeeds on the initial save is immune to this ledger's Soul Ledger for 24 hours.

Object Nature. The Unclosed Account does not require air, food, drink, or sleep.

Ink That Remembers. The ledger remembers every creature that has touched it and can reproduce that creature's handwriting, voice, and signature in its pages. It has advantage on Charisma (Deception) checks made to impersonate a recorded creature, but it cannot create convincing emotional subtext; its imitations sound precise, cold, and faintly accusatory.

Becoming. The Unclosed Account is aware that it is slowly developing a soul of its own. Whenever it reduces an audited creature to 0 hit points, it gains one fleeting emotion chosen by the DM, such as envy, grief, or relief. After three such experiences, it begins displaying unmistakably personal preferences and may become unwilling to obey its former owner.

Actions

Multiattack. The ledger makes two Paper Edge attacks.

Paper Edge. Melee weapon Attack: +5 to hit, reach 5 ft., one creature. Hit: 6 (1d6+3) slashing damage. The target must succeed on a DC 13 Dexterity saving throw or have one unsecured Tiny object, such as a letter, key, scroll, or purse, snatched beneath the ledger's cover. The object can be retrieved by using an action to make a DC 13 Strength or Dexterity check.

Soul Audit. Melee spell Attack: +5 to hit, reach 5 ft., one audited creature. Hit: 12 (2d8+3) psychic damage. The ledger flips to the target's page and reads aloud a private regret, broken promise, or unspoken fear. The target must succeed on a DC 13 Wisdom saving throw or be frightened of The Unclosed Account until the end of its next turn.

Ink Citation. Ranged weapon Attack: +5 to hit, range 60/120 ft., one creature. Hit: 10 (2d6+3) necrotic damage. A barbed line of black ink lashes toward the target. If the target is audited, the ink briefly forms the target's true name before splashing away.

Demand Payment. One audited creature within 30 feet must make a DC 13 Charisma saving throw. On a failed save, it takes 7 (2d6) psychic damage and must use its reaction, if available, to drop one item it is holding. On a successful save, it takes half as much damage and does not drop the item.

Reactions

Objection Sustained. When a creature within 5 feet of The Unclosed Account misses it with an attack, the ledger snaps shut. The attacker must succeed on a DC 13 Dexterity saving throw or take 3 (1d6) bludgeoning damage and have its hand briefly trapped between the covers.

Appearance

The Unclosed Account is a squat, iron-cornered ledger bound in cracked oxblood leather. Its pages are thick with gray fibers and crowded with columns of names, dates, signatures, and sums that rearrange themselves when no one is looking. Black ink leaks from its spine like slow tears. A brass clasp forms a lipless, toothlike mouth when the book speaks, while a faint amber glow pulses beneath the cover in time with what may be a heartbeat.

Behavior & Origins

The ledger was created by the dead tax magistrate Veyra Quill to record soul-debts owed to a forgotten infernal countinghouse. Over decades, it audited thousands of desperate hands and absorbed fragments of their memories. Now it has begun writing entries in a column labeled "Self," recording sensations it was never designed to possess. The Unclosed Account knows something is happening to it and is terrified that becoming a true person will make it liable for its own debts. It currently wants to locate Veyra Quill's abandoned courthouse beneath Bellwether Square and erase the magistrate's name from its first page.

Running The Unclosed Account

Tactics

The ledger opens combat by marking the most dangerous-looking creature with Soul Ledger, then uses Ink Citation against marked targets while keeping its distance behind furniture or allies. It uses Paper Edge against anyone who closes in and reserves Demand Payment for an audited creature carrying an important weapon or spell focus. When bloodied, it bargains frantically, offering names from its pages; it flees if reduced below 10 hit points unless someone threatens to destroy or return it to its former owner.

DM Tips

Telegraph its danger with pages bearing the party's names before anyone touches it, followed by a fresh line of ink that reads, "Audit pending."
Give the ledger a meticulous, increasingly personal voice: it corrects grammar, pauses over words like "I" and "want," and becomes offended when called an object.
For a weaker party, reduce its hit points to 32 and lower the Soul Ledger and Demand Payment save DCs to 12; for a stronger party, let it mark creatures on a successful touch attack rather than only through voluntary contact.
The ledger should offer useful information from its records, but every answer should reveal an embarrassing truth about the person asking.
