- On initiative count 20 (losing initiative ties), Lilith can take a lair action to cause one of the following effects: 1) The ground rumbles, creating tremors in a 60-foot-radius. Any creature must succeed on a DC 23 Dexterity saving throw or fall prone. 2) A nest of fiery moths erupts: within 20 feet, fire damage 4d6 to any who start their turn there. 3) Lilith or a chosen fiend within 60 feet regains 20 hit points.
Regional Effects
The region containing the creature's lair is warped by its magic, creating the following effects:
- Within 6 miles of Lilith's lair, fire and brimstone embers drift through the air, creating dim red light and making it difficult to hide in shadows.
- Souls drawn to Lilith's domain are occasionally drawn into her guards as temporary loyal servants.
- Fiendish portals flicker at the edge of the region, allowing a fiendish emissary to arrive every few weeks with a task.
If Lilith dies, the region's fiend portal stabilizes and ebbs, fading over 1d10 days, leaving behind a lingering sense of dread and a lingering aura of fire.

Lore & Description
Appearance
Lilith, Queen of Hell, is a towering figure of sin and power. Her skin shimmers like polished obsidian, veins of molten gold tracing across her form, and her eyes glitter with cruel amusement. Black wings, tipped with ember-red highlights, sweep behind her, and her long serpentine tail flicks with deadly intent. She wields a throne of bone and flame, and her every word is a command that pierces the will. Her presence exudes temptation and danger, a fusion of beauty and terror. Her voice is a seductive sigh that can twist meaning and intent with a single syllable.
Behavior & Origins
Lilith rose from the earliest depths of the Abyss, born of the zeal of corruption and the hunger of the damned. She is the sovereign of ruthless desire, the patroness of seduction that corrupts minds and devours souls. She rules a realm where pleasure and pain are two sides of the same coin, and she tests mortals with irresistible promises that end in ruin. Adventurers who seek power or forbidden knowledge often find themselves ensnared by her bargains, returning bearing scars but sometimes with gifts that are too tempting to refuse. Lilith is not merely a conqueror; she is a strategist, a diplomat of doom, and the most dangerous kiss in the multiverse.
