Lore & Description
Appearance
This animate quilt is sewn from mismatched hospital blankets: yellowed wool, faded blue cotton, and strips of gray flannel marked with old quarantine symbols. Its seams twitch like tendons, and dozens of rusted buttons form a suggestion of staring eyes. Heat radiates from its folds, carrying the smell of fever, vinegar, and rain-soaked ash. It moves by dragging itself along the floor or clinging to walls like a sluggish, many-legged thing.
Behavior & Origins
The quilt was made from blankets discarded by the Ashcross Plague Ward after its final night, when every patient and healer died before dawn. A desperate protective ritual stitched into the ward's linens awakened it, but the ritual never told the quilt that the patient it guards is gone. It now protects an empty cot in the ruined ward, attacking anyone who approaches while periodically smoothing the blankets and whispering through its seams. The quilt's patient was called Mara Venn, and her missing brass identification tag is still hidden in the cot's straw mattress.

Tactics
The quilt begins by placing itself between intruders and the nearest bed, using Guard the Empty Bed to make itself difficult to dislodge. It then attempts Smothering Fold against the smallest or weakest-looking creature before battering anyone who tries to free the victim. If reduced to 3 hit points or fewer, it retreats toward the empty cot and fights desperately beside it; it flees only if the cot is destroyed or carried away.
DM Tips
- Telegraph the creature with a warm patch of floor, freshly smoothed blankets, and tiny wet footprints leading toward an apparently empty cot.
- Roleplay it through physical habits rather than speech: smoothing sheets, tucking in unconscious creatures, and recoiling violently when someone disturbs the bed.
- For a weaker party, remove its cold resistance and lower Smothering Fold's save DC to 10; for a stronger party, give it two quilts as attendants or let the empty cot animate as difficult terrain.
Loot
- Mara Venn's Brass Tag — A tarnished identification tag hidden in the quilt's protected cot. It bears the name of a plague victim whose family still believes she escaped.
- Ashcross Ward Blanket — A square of clean, unnaturally warm fabric cut from the quilt. Wrapped around a sleeping creature, it grants advantage on the next saving throw against disease, then becomes cold and ordinary.
