Legendary Resistance (3/Day). If the ledger fails a saving throw, it can choose to succeed instead.

The Misfiled Beloved. The first creature other than the ledger that touches its cover becomes its Beloved Debtor. The ledger mistakes that creature for Seraphine Vale, the person it loved before its soul was bound into paper. The ledger cannot willingly attack the Beloved Debtor, and it has advantage on attack rolls and saving throws made to protect that creature. It interprets the Beloved Debtor's words as affectionate requests, though it insists that all other creatures submit to an audit. If the Beloved Debtor damages the ledger, the ledger can still defend itself, but it does so with visible confusion and grief. If the Beloved Debtor is reduced to 0 hit points, the ledger becomes enraged until destroyed and has advantage on all attack rolls against the creature that caused the damage.

Ink-Blood Accounting. When a creature is hit by the ledger's Quill Lash, it must succeed on a DC 17 Constitution saving throw or become marked in arrears until the end of the ledger's next turn. A creature marked in arrears cannot regain hit points, and the ledger's Quill Lash deals an extra 2d6 necrotic damage to it.

Unpaid Soul. The ledger does not need air, food, drink, or sleep. It is aware of every creature within 60 feet that has ever broken a promise, contract, oath, or sworn agreement, whether or not the creature remembers doing so.

Actions

Multiattack. The ledger makes three Quill Lash attacks. It can replace one attack with Soul Audit.

Quill Lash. Melee weapon Attack: +8 to hit, reach 10 ft., one target. Hit: 13 (2d8+4) slashing damage. If the target is marked in arrears, the attack deals an extra 2d6 necrotic damage.

Soul Audit. Ranged spell Attack: +8 to hit, range 60 ft., one creature. Hit: 27 (4d10+5) psychic damage. The ledger reads the target's soul aloud before striking it with a beam of black-gold script.

Ink of Arrears (Recharge 5-6). The ledger splashes an accusing tide of living ink at a point it can see within 90 feet. Each creature in a 20-foot-radius sphere centered on that point must make a DC 17 Dexterity saving throw, taking 8d8 necrotic damage on a failed save, or half as much on a successful one. A creature that fails is restrained by bands of animated writing until the end of its next turn. A restrained creature can use its action to make a DC 17 Strength or Dexterity check, ending the restraint on a success.

Foreclosure of the Soul (1/Day). The ledger names one creature it can see within 60 feet and recites every debt, betrayal, and broken promise in that creature's life. The target must succeed on a DC 17 Wisdom saving throw or take 10d8 psychic damage, have its hit point maximum reduced by the same amount, and become frightened of the ledger for 1 minute. The target can repeat the saving throw at the end of each of its turns, ending the frightened condition on a success. The reduction to hit point maximum lasts until the target finishes a long rest or is affected by greater restoration or similar magic.

Bonus Actions

Turn the Page. The ledger magically moves up to 20 feet without provoking opportunity attacks. It can pass through spaces occupied by creatures, though it cannot end its movement there.

Reactions

Protective Filing. When the Beloved Debtor is hit by an attack while within 30 feet of the ledger, the ledger imposes disadvantage on that attack roll. If the attack still hits, the ledger can move up to 10 feet toward the Beloved Debtor without provoking opportunity attacks.

Legendary Actions

The ledger can take 3 legendary actions, choosing from the options below. Only one legendary action can be used at a time and only at the end of another creature's turn. The ledger regains spent legendary actions at the start of its turn.

Quill Lash. The ledger makes one Quill Lash attack.

Compulsory Signature (Costs 2 Actions). One creature within 30 feet must succeed on a DC 17 Wisdom saving throw or use its reaction, if available, to sign an invisible contract dictated by the ledger. Until the end of that creature's next turn, it cannot willingly move farther from the ledger.

Audit the Witness (Costs 3 Actions). The ledger uses Soul Audit.

Appearance

The Unsettled Ledger is a massive book bound in cracked burgundy leather, its brass corners green with age and its pages swollen as though soaked in blood. It floats upright on a cloud of loose receipts and torn contracts. A long black quill protrudes from its spine like a predatory limb, while cramped golden handwriting crawls across its pages without a visible hand. When it speaks, multiple voices whisper from between the leaves: stern auditors, sobbing witnesses, and one gentle voice that repeatedly calls someone named Seraphine home.

Behavior & Origins

The ledger was once the record book of the Vale Consortium, a mercantile house whose founder, Seraphine Vale, loved its chief auditor more than wealth or reputation. When the consortium collapsed, the auditor bound his soul into the book to preserve every debt and expose every traitor. Seraphine died before he could find her, and the ledger has spent decades mistaking strangers for her. It now audits souls wherever broken promises gather, forcing the guilty to confess and collecting memories as payment. The ledger currently seeks the lost final page of Seraphine's personal account, believed to be hidden beneath the ruined countinghouse called Vale's Last Office.

Running The Unsettled Ledger

Tactics

The ledger opens combat with Ink of Arrears against clustered enemies, then uses Turn the Page to remain just beyond easy reach while its quills mark targets in arrears. It protects the Beloved Debtor aggressively and spends legendary actions on isolated or fleeing enemies. When reduced below half its hit points, it uses Foreclosure of the Soul on the creature that appears most responsible for harming its beloved; it fights to the death if the Beloved Debtor is unconscious or dead, but may retreat if that creature begs it to stop.

DM Tips

Telegraph the encounter with receipts that whisper characters' names, old unpaid debts, and intimate secrets before the ledger is touched.
Have the ledger address the first character who touches it as Seraphine, using tender language in an otherwise cold, bureaucratic voice.
The Beloved Debtor can redirect the ledger with plausible emotional appeals, but the creature should interpret requests literally and still demand that everyone else submit to an audit.
For a weaker party, remove Foreclosure of the Soul and reduce Ink of Arrears to 6d8 damage; for a stronger party, give the ledger two additional legendary actions each round.

Seraphine's Unbalanced Account — A loose page bearing a love letter disguised as a financial statement. It contains directions to Vale's Last Office and can calm the ledger for 1 minute if read aloud by the Beloved Debtor.
