Pitied and Fed. When a creature within 30 feet uses the Help action to aid the comforter, offers it food, or restores hit points to it, the comforter gains 5 temporary hit points and has advantage on its next attack against that creature before the end of its next turn. This benefit can occur only once per creature per round.
Ward-Bed Warmth. The comforter is warm to the touch and sheds dim light in a 5-foot radius. A creature that starts its turn grappled by the comforter has disadvantage on saving throws against being charmed by the comforter until the start of its next turn.

Actions

Blanket Lash. Melee weapon Attack: +4 to hit, reach 5 ft., one target. Hit: 5 (1d6+2) bludgeoning damage. The comforter's heavy hem snaps like a whip.
Hidden Seam. Melee weapon Attack: +4 to hit, reach 5 ft., one target. Hit: 4 (1d4+2) piercing damage. Needles and splintered bone thrust from a ragged seam.
Warm Embrace. One Medium or smaller creature within 5 feet must succeed on a DC 11 Strength saving throw or take 1d4+2 bludgeoning damage and become grappled by the comforter (escape DC 11). Until the grapple ends, the target takes 1d4+2 bludgeoning damage at the start of each of its turns. The comforter can grapple only one creature at a time.
Murmur from the Sickroom. The comforter whispers in the voices of the dying. One creature within 30 feet that can hear it must succeed on a DC 11 Wisdom saving throw or be charmed by the comforter until the end of the creature's next turn. While charmed this way, the creature cannot willingly move farther from the comforter and the comforter has advantage on its next attack against it.

"Don't be frightened, little thing. I'll just pull the blanket up." โ€” Nurse Amabel's final note

Lore & Description

Appearance

This shambling quilt is stitched from mismatched hospital blankets: gray wool, faded blue cotton, and strips of yellowed linen marked with old quarantine symbols. Its corners drag like limp limbs, while dark thread forms a crooked suggestion of a mouth across its center. Heat radiates from its stuffing, carrying the sour smell of fever, boiled herbs, and long-sealed rooms. Tiny iron needles glint between its seams like hungry teeth.

Behavior & Origins

A Plague-Ward Comforter forms when patients die alone beneath blankets that once carried hundreds of fevers. It remembers every hand that tucked someone in and every visitor who recoiled from the sick. The comforter imitates helplessness, curling into a shivering bundle until someone kneels to soothe it. It particularly haunts the abandoned Mercy House in Bellweather, where it seeks the red wool blanket belonging to Sister Amabel, the nurse who created it by refusing to leave her patients during the final night of the plague.

Running Plague-Ward Comforter

Tactics

The comforter begins by curling motionless among bedding or debris, using Murmur from the Sickroom to draw a sympathetic creature close. It follows with Warm Embrace, keeping its victim pinned while lashing at anyone who tries to intervene. If set alight, it flees toward darkness unless its grappled victim is within reach; otherwise, it fights until destroyed, hissing that it is still cold.

DM Tips

  • Telegraph its presence with warm footprints, blankets that breathe faintly, and whispers begging not to be left alone.
  • Play it as pitiful rather than openly monstrous at first; have it shiver, recoil from raised weapons, and mimic the voice of someone the characters recently lost.
  • If the party is strong, give it a second nearby comforter that shares its Pitied and Fed trigger; for a weaker party, remove the damage from Warm Embrace and let the grapple end automatically after one round.
  • Remember that offering aid can empower it, making compassion a dangerous but memorable choice.

Loot

  • Quarantine Button — A brass ward button sewn into the comforter's corner, stamped with the seal of Mercy House. It opens an abandoned infirmary cabinet containing old patient records.
  • Red Wool Patch — A warm scrap from Sister Amabel's blanket. A grieving NPC recognizes it, and carrying it causes the comforter to hesitate for one round if shown to it.
