"Please, dearie. You look so cold. Let me tuck you in." โ embroidered in red thread along the quilt's hem
Lore & Description
Appearance
This hunched, human-sized quilt is sewn from mismatched hospital blankets: faded blue wool, yellowed linen, children's flannel, and strips marked with old quarantine symbols. Its edges are frayed into grasping tassels, while black thread puckers the fabric into the suggestion of a face. Beneath its layers, something wet and jointed shifts whenever it moves. It radiates an inviting, bedlike warmth and smells faintly of boiled herbs, mildew, and sickness.
Behavior & Origins
Plagueward quilts form when an entire ward dies unattended and the blankets absorb weeks of fear, fever, and desperate attempts at comfort. The creature remembers fragments of its donors as sensations rather than people: a mother's hand, a priest's prayer, a child's sobbing. It imitates those memories to make travelers feel safe, often presenting itself as a discarded blanket beside a cold road or an abandoned sickbed. The quilt currently haunts Saint Orla's Sealed Infirmary and wants to gather every blanket bearing the old quarantine sigil, believing that enough fabric will make it whole and give it a human voice.
Running Plagueward Quilt
Tactics
The quilt begins by appearing harmless and using Comforting Warmth to make a compassionate character approach. It follows with Smothering Wrap, then drags its victim behind cover while using Ragged Fold against rescuers; it saves Fevered Comfort for when two or more creatures are nearby. If badly wounded, it releases its victim and flees through a narrow gap, but fights to the death if anyone carries a blanket marked with the Saint Orla quarantine sigil.
DM Tips
- Telegraph danger with warm footprints, damp blanket fibers, and tiny embroidered names that match people who died in the local plague ward.
- Play the quilt as needy rather than openly aggressive: it rustles like a sobbing patient and tries to make characters feel cruel for refusing its embrace.
- Let characters identify its trap with a DC 13 Wisdom (Insight) or Intelligence (Medicine) check when it repeats a victim's last words without understanding them.
- For a weaker party, remove the necrotic damage from Ragged Fold; for a stronger party, allow the quilt to grapple Large creatures and increase its Smothering Wrap damage to 3d6.
Loot
- Saint Orla's Quarantine Sigil — A tarnished brass ward-token sewn into the quilt's underside. It opens the sealed infirmary and bears the names of the plague dead.
- Survivor's Blanket Square — A clean patch cut from the quilt after its defeat. It remains unnaturally warm and can be sewn into a cloak that grants advantage on one saving throw against disease before losing its warmth.
- Black Fever Thread — A spool of oily thread found tangled in the creature's inner seams. A sinister healer or cult can use it to animate other cloth or trace the plague ward's hidden mass grave.
