Appearance
This animated quilt is assembled from mismatched blankets: faded blue hospital wool, yellow children's flannel, red-striped army cloth, and scraps of bedsheets marked with old quarantine symbols. Its seams are thick with black thread, and its corners twitch like searching fingers. Warmth radiates from its patchwork surface despite the cold, carrying the smells of boiled linen, bitter medicine, and long-vanished sickness. When it moves, the quilt folds and billows as though someone invisible were struggling beneath it.
Behavior & Origins
The Wardpatch Quilt was sewn by nurses during the Ashbell Fever in Saint Orra's plague ward, each square cut from a blanket belonging to a patient who died there. The final stitch was made by Matron Elian Voss, who whispered that every patient deserved warmth and every coward deserved consequences. Now the quilt wanders abandoned infirmaries and tenements, seeking the lost brass bell of Saint Orra's ward. It will gladly accept a fair duel, but it hunts those who prey on the helpless.

Tactics
The quilt opens with Smother against the most aggressive nearby combatant, then uses its bulk to keep that creature isolated while lashing out with Slam attacks. It uses Ward's Rebuke whenever an enemy attacks a helpless ally, often drifting between combatants to punish such behavior. When reduced below half its hit points, it uses Flap and Scatter to create space and retreats toward a doorway, sickroom, or other place where it can force the party to fight fairly; it flees if its target agrees to stop attacking the helpless.
DM Tips
- Telegraph the quilt with warm footprints in dust, blankets folded into neat hospital corners, and old warning bells ringing without wind.
- Play it as stern rather than mindless: it straightens its seams, points accusingly at dishonorable tactics, and pauses to let a fallen combatant stand.
- Its fair-fight rule works best when announced aloud; have it whisper the rule before using Ward's Rebuke.
- For a weaker party, remove Flap and Scatter; for a stronger party, let the quilt use Smother against Large creatures and increase its save DCs to 13.
Loot
- Saint Orra's Brass Ward Bell — A tarnished handbell sewn into the quilt's inner lining. Ringing it once grants advantage on the next saving throw against disease, but it also attracts the quilt from up to a mile away.
- Quarantine Ribbon — A faded black-and-yellow ribbon bearing Matron Voss's initials. A healer who wears it can identify whether a creature is poisoned or diseased by touch.
